IMPORTANT INFORMATION FOR NEW PATIENTS: Federal law requires all healthcare practices to obtain, verify, and record information that identifies each new patient. WHAT THIS MEANS TO YOU: When you open/create an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We will also ask to see your driver's license or other identifying documents, i.e. military ID, passport, or other government issued ID. Thank you for your cooperation.”

Your evaluation appointment will consist of a consultation explaining your diagnosis and treatment options. If necessary, a treatment appointment will be scheduled at that time.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any X-rays, if applicable.
  • A list of medications, with dosage, you are presently taking, including all over-the-counter medications.
  • If you have dental insurance, bring the necessary insurance cards. When you schedule your appointment, our patient care coordinators will ask you for this information.

IMPORTANT: All patients 18 years of age and under must be accompanied by a parent or legal guardian at the consultation visit. If you are over 18 years of age, and wish to have other persons aware of your dental treatment, you will need to provide a statement as to whom we are able to share your confidential dental treatment plan with.

A pre-operative consultation and clinical examination is mandatory for patients requiring medication for dental anxiety. Your attending doctor will determine what medication is necessary and prescribe the appropriate dosage.

Please alert the office prior to treatment if you have a medical condition, which may require special attention (i.e. inmmunocompromised, diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.), if you are on medication (i.e. heart medications, aspirin, anticoagulant therapy (Coumadin or Plavix)), or if you use a wheelchair, walker or are pregnant.
